Marchildon collects the personal information necessary to provide its orthopedic services, including your name, address, postal code, phone number, email address, health insurance number, medical information relevant to your treatment, and details related to your public or private insurance plans.

Information is collected through forms on our website, our appointment forms, in person at one of our clinics, by phone, or by email.

2. Use of information

Your personal information is used to provide orthopedic services, manage your patient file, billing and claims with payer organizations (RAMQ, CNESST, SAAQ, Veterans Affairs Canada, Indigenous Services Canada, private insurers), and for communication regarding your appointments, follow-ups, and renewals.
